Thioridazine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Berberine interacts with.

Theoretically, berberine might increase serum levels of drugs metabolized by CYP2D6.
In vitro research and preliminary clinical evidence show that berberine can inhibit CYP2D6. Theoretically, use of berberine with drugs metabolized by CYP2D6 might increase drug levels and increase the risk of adverse effects.

Here's the concern with pairing these two. Your body relies on a liver enzyme called CYP2D6 to clear thioridazine out of your system. Berberine appears to slow that enzyme down, so thioridazine could build up to higher levels than expected. Since thioridazine is not a prodrug, more drug hanging around means a stronger effect and a greater chance of side effects.

This matters with thioridazine because it can affect the heart's rhythm (QT prolongation). Higher levels could raise that risk, along with drowsiness, dizziness, and dry mouth. The evidence here is still fairly limited and theoretical, but given thioridazine's heart risks, I'd want you to check with your pharmacist or doctor before adding berberine rather than guessing.

What Is Thioridazine Used For?

Brand name: Mellaril

Thioridazine Hydrochloride tablets are used for the management of schizophrenia — but only in patients who have already tried at least two other antipsychotic medications without adequate results, either because those drugs didn't work well enough or caused intolerable side effects.

Because thioridazine carries serious heart-rhythm risks, it is considered a last-resort option for schizophrenia, not a first-line treatment. It is not approved for dementia-related psychosis or any other psychiatric condition.
