Trazodone HCl falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Berberine interacts with.

Theoretically, berberine might increase serum levels of drugs metabolized by CYP2D6.
In vitro research and preliminary clinical evidence show that berberine can inhibit CYP2D6. Theoretically, use of berberine with drugs metabolized by CYP2D6 might increase drug levels and increase the risk of adverse effects.

Here's the gist if you take trazodone (Trazorel) and you're thinking about adding berberine. Your body partly relies on an enzyme called CYP2D6 to clear trazodone out. Berberine appears to slow that enzyme down, so the medicine may not break down as quickly and could build up a bit in your system. Since trazodone is active as-is (not a prodrug), more of it hanging around tends to mean stronger effects.

In practice that could show up as extra drowsiness, dizziness, low blood pressure when you stand up, or a groggy, hungover feeling. The good news is this is mostly theoretical, based on lab work and early studies, so it may not happen at all. Still, please run it by your pharmacist or doctor before starting berberine.

Here's the simple version: your body relies on a liver enzyme called CYP3A4 to clear trazodone out of your system. Berberine appears to slow that enzyme down a bit. So when you take the two together, trazodone may hang around longer and build up to higher levels than usual.

What might that feel like? Mostly an exaggerated version of trazodone's known effects, more drowsiness, dizziness, low blood pressure when you stand up, or a groggy, hungover feeling in the morning.
