Bitter Melon and Ethinyl Estradiol, Drospirenone

Here is the interaction we found between these two, with the evidence behind it.

Moderate

P-GLYCOPROTEIN SUBSTRATES

Why this applies to Ethinyl Estradiol, Drospirenone

Ethinyl Estradiol, Drospirenone falls under P-GLYCOPROTEIN SUBSTRATES — the group Bitter Melon interacts with.

Theoretically, bitter melon might increase levels of P-glycoprotein substrates.
Bitter melon might inhibit the p-glycoprotein (P-gp) intestinal pump and increase intracellular levels of P-gp substrates. In vitro research in intestinal cells shows that 1-monopalmitin, a constituent of bitter melon, increases levels of daunomycin, a P-gp substrate. Additionally, drinking bitter melon juice has been associated with a case of acute pancreatitis in a patient who had been taking pazopanib, a P-gp substrate, for 8 years. Researchers theorize that inhibition of P-gp led to increased levels of pazopanib, resulting in pazopanib-induced pancreatitis.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for your Yaz (ethinyl estradiol with drospirenone). Bitter melon may block a tiny intestinal pump called P-glycoprotein, which can let a bit more of certain drugs slip into your body. In theory that could raise the levels of your birth control hormones slightly, which might nudge up side effects like nausea, breast tenderness, bloating, or mood changes.

I want to be honest about how soft this evidence is. It comes from lab and animal studies, plus one unrelated case report, not from real-world testing with birth control. Eating bitter melon as an occasional food is unlikely to cause trouble. Concentrated supplements are the bigger question mark, so it's worth a quick chat with your pharmacist before adding one.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.

About Bitter Melon

What is Bitter Melon?

Bitter Melon
  • Blood sugar support
  • Diabetes
  • Digestive complaints
  • Antioxidant source
  • Skin conditions

Bitter melon is a tropical fruit eaten as food and used in traditional medicine, most often for blood sugar control. While some small studies hint it may modestly lower blood sugar, the evidence is limited and inconsistent, so it should not replace proven diabetes treatment. Talk with your pharmacist or doctor before using it, especially if you take diabetes medicines or are pregnant.
