Here's the simple version: both black cohosh and fenofibrate can, on rare occasions, be hard on your liver. Fenofibrate is known to sometimes bump up liver enzymes, and black cohosh has a handful of reported cases of liver trouble too. The worry is that putting them together might add to that strain. This isn't about one changing the other's blood levels, it's just two things that occasionally irritate the liver sharing the same load.

I want to be honest about the evidence here: this is mostly theoretical and based on scattered case reports, not strong studies, so please don't panic. Still, watch for signs like yellowing eyes or skin, dark urine, belly pain, or unusual tiredness, and check with your pharmacist or doctor before combining them, especially if you already get liver labs done.

Black cohosh is a North American plant most often used to ease menopause symptoms like hot flashes, but the research is mixed and far from settled. It is generally well tolerated for short-term use, though rare reports of liver problems mean you should talk with your pharmacist or doctor before trying it, especially if you have liver disease.

About Fenofibrate

What Is Fenofibrate Used For?

Brand names: Tricor, Fenofibrate, Lipidil Micro, Lipidil Supra, Antara, Lofibra, Triglide, Lipidil EZ

Fenofibrate is used alongside a cholesterol-lowering diet in adults to improve abnormal lipid levels. Tricor (tablet) and fenofibrate tablets are indicated to lower LDL cholesterol (bad cholesterol), total cholesterol, triglycerides, and apolipoprotein B, and to raise HDL cholesterol (good cholesterol) in adults with primary hypercholesterolemia (high cholesterol) or mixed dyslipidemia (mixed abnormal lipid levels). They are also used to treat severe hypertriglyceridemia (very high triglycerides).

Antara and Lipofen (capsules) are indicated to reduce triglyceride levels in adults with severe hypertriglyceridemia, and to lower LDL cholesterol in adults with primary hyperlipidemia when other recommended LDL-lowering therapies cannot be used. Fenofibrate has not been shown to reduce heart attacks or cardiovascular deaths in patients with type 2 diabetes.
