Here's the simple version. Both black cohosh and nitrofurantoin have, on rare occasions, been connected to liver trouble. Nitrofurantoin is well known to cause liver irritation in a small number of people, especially with longer courses, and black cohosh has a handful of reports linking it to liver problems too. The worry is that using them together might add up and raise your risk a little.

I want to be honest, though: this is based on scattered case reports, not strong studies, so for most people taking a short antibiotic course it likely won't be an issue. Still, watch for warning signs like yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and check with your pharmacist or doctor before combining them, especially if you have any liver concerns.

Black cohosh is a North American plant most often used to ease menopause symptoms like hot flashes, but the research is mixed and far from settled. It is generally well tolerated for short-term use, though rare reports of liver problems mean you should talk with your pharmacist or doctor before trying it, especially if you have liver disease.

About Nitrofurantoin

What Is Nitrofurantoin Used For?

Brand names: Macrodantin, Furadantin, Furatoin, Macrobid

Nitrofurantoin is used to treat urinary tract infections (UTIs) caused by certain susceptible bacteria. The capsule forms (including Macrodantin) are indicated for UTIs due to susceptible strains of Escherichia coli, enterococci, Staphylococcus aureus, and certain strains of Klebsiella and Enterobacter species. Nitrofurantoin oral suspension (Furadantin) is indicated for adults and children 1 month of age and older for the same types of UTIs.

Nitrofurantoin is not used for kidney infections (pyelonephritis) or infections of the tissue around the kidney (perinephric abscesses). Because it works primarily in the urine and does not spread widely through body tissues, it may not fully clear the infection in some patients, and the UTI may return after treatment.
