Gemifloxacin falls under ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP) — the group Black Cohosh interacts with.

Black cohosh may inhibit one form of OATP, OATP2B1, which could reduce the bioavailability and clinical effects of OATP2B1 substrates.
In vitro research shows that black cohosh modestly inhibits OATP2B1. OATPs are expressed in the small intestine and liver and are responsible for the uptake of drugs and other compounds into the body. Inhibition of OATP may reduce the bioavailability of oral drugs that are substrates of OATP.

Here's the gist with gemifloxacin (Factive) and black cohosh. Gemifloxacin is partly absorbed through a gut transporter called OATP2B1, which acts like a doorway that ushers the drug from your intestine into your bloodstream. Lab studies suggest black cohosh may modestly block that doorway. In theory, that could mean a bit less of your antibiotic gets absorbed, which matters because an antibiotic needs to reach high enough levels to fully clear an infection like pneumonia.

I want to be honest, though: this is graded as a minor interaction, and the evidence comes only from test-tube research, not real people. For most folks it likely won't cause a problem. Still, since finishing antibiotics strong is important, mention the black cohosh to your pharmacist or doctor so they can keep an eye on how you're doing.

  • Gemifloxacin is used to treat pneumonia. Gemifloxacin may also be used to treat bronchitis but should not be used for this condition if there are other treatment options. Gemifloxacin is in a class of antibiotics called fluoroquinolones. It works by killing bacteria that cause infections.

  • Antibiotics such as gemifloxacin do not work for colds, flu, or other viral infections. Using antibiotics when they are not needed increases your risk of getting an infection later that resists antibiotic treatment.
