Here's the honest picture if you're using estradiol (whether that's a patch, vaginal cream, ring, or tablet) and thinking about black cohosh. On paper, black cohosh has been looked at for possible estrogen-like activity, so the concern is that it might either add to or work against your estradiol. But this worry comes mostly from lab and test-tube studies, not from solid human evidence, and most modern research suggests black cohosh doesn't really act like estrogen in the body.

So I wouldn't lose sleep over it, but I'd stay alert. If you notice things like breast tenderness, spotting, bloating, or your menopause symptoms shifting, mention it. Since black cohosh has rare liver concerns, it's worth a quick chat with your pharmacist or doctor before combining them.
