Morphine Sulfate falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Black Cohosh interacts with.

Some research suggests that black cohosh might inhibit CYP2D6, but there is conflicting evidence.
Some clinical research suggests that black cohosh might modestly inhibit CYP2D6 and increase levels of drugs metabolized by this enzyme. However, contradictory clinical research shows a specific black cohosh product (Remifemin, Enzymatic Therapy) 40 mg twice daily does not significantly inhibit metabolism of a CYP2D6 substrate in healthy study volunteers. Until more is known, use black cohosh cautiously in patients taking drugs metabolized by CYP2D6.

Here's some good news if you're reaching for black cohosh during menopause while also taking morphine: this pairing isn't a big worry. The interaction on file is about black cohosh possibly slowing down a liver enzyme called CYP2D6. But morphine isn't really cleared by CYP2D6 in a way that matters much. It's mostly handled by a different pathway (glucuronidation), so even if black cohosh nudges CYP2D6, your morphine levels shouldn't shift in a meaningful way.

On top of that, the evidence here is shaky and conflicting, with one product showing no real effect at all. That said, if you ever feel unusually drowsy, foggy, constipated, or notice slow breathing, contact your doctor. It's always smart to run new supplements by your pharmacist first.

Morphine is used to manage acute (short-term) and chronic (long-term) pain that is severe enough to need an opioid and that hasn't been adequately treated by other options. The higher-concentration liquid form is reserved for adults who are already opioid-tolerant — meaning their body is used to opioid medications.

Because of real risks of addiction, misuse, and overdose, morphine is only prescribed when other pain treatments have not worked or are not expected to work. It is not intended for long-term use unless the pain continues to be severe and no alternatives are suitable.
