Why this applies to Celiprolol
Celiprolol falls under ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP) — the group Black Cohosh interacts with.
In vitro research shows that black cohosh modestly inhibits OATP2B1. OATPs are expressed in the small intestine and liver and are responsible for the uptake of drugs and other compounds into the body. Inhibition of OATP may reduce the bioavailability of oral drugs that are substrates of OATP.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for celiprolol, which is a beta-blocker used to manage high blood pressure. Your gut relies on a little transporter called OATP2B1 to pull celiprolol from your intestine into your bloodstream. In lab studies, black cohosh appears to mildly block that transporter. If less of the drug gets absorbed, you might get a bit less blood pressure control than expected.
The good news: this is rated a minor interaction, and the only evidence so far comes from test-tube work, not real people. So I wouldn't lose sleep over it. Still, if you take black cohosh and notice your blood pressure creeping up, mention it to your pharmacist or doctor so they can keep an eye on things.
