Acetaminophen, Butalbital, Caffeine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Black Pepper interacts with.
In vitro research and pharmacokinetic simulation data suggest that piperine, a constituent of black pepper, as well as the pepper fruit seem to inhibit CYP3A4. This has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with your headache combo (Tecnal, Esgic, and similar). One piece of this pill, butalbital, is a barbiturate that your liver clears partly through an enzyme called CYP3A4. Concentrated black pepper supplements (the piperine extract) can slow that enzyme down in lab studies. In theory, that could let a bit more butalbital linger in your body, which might mean more drowsiness, dizziness, or that foggy, sluggish feeling. None of these medicines is a prodrug, so there's no flip in direction here.
I'll be honest: this is only seen in test-tube and computer-modeled data, not in real people, so it's more of a heads-up than a red flag. A sprinkle of pepper on your food is no concern. If you take piperine-rich supplements, just run it by your pharmacist first.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for your headache combo (Esgic, Fiogesic, and similar). The caffeine piece of this medication is partly cleared by a liver enzyme called CYP1A2. Lab studies hint that concentrated black pepper extract (piperine) may speed up that enzyme, which could in theory clear the caffeine a bit faster and slightly soften its effect. Importantly, this has only been seen in test-tube and animal work, never in real people.
So I wouldn't lose sleep over it. A sprinkle of pepper on your food is a non-issue. If you take a strong piperine supplement (often paired with turmeric) and notice your tension-headache relief feels weaker, just mention it to your pharmacist or doctor and we can sort it out together.

This combination product — available as capsules, tablets, and oral solution under various names including Fioricet — is used to relieve the symptoms of tension headaches (also called muscle contraction headaches).
