Theoretically, black pepper might increase the risk of hypoglycemia when taken with antidiabetes drugs.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's something worth knowing about this pairing, though I'll be honest up front: it's mostly theoretical. Dasiglucagon (Zegalogue) is your rescue medicine for severe low blood sugar. It tells your liver to dump stored sugar into your blood to bring you back up fast. Black pepper, especially the concentrated piperine in supplements, has shown in animal studies that it can nudge blood sugar a bit lower.

So in theory, a lot of piperine could work slightly against what dasiglucagon is trying to do, or push your sugar low between emergencies. That said, the pepper you sprinkle on dinner is very unlikely to matter. If you take piperine supplements regularly, mention it to your pharmacist or doctor so they can keep an eye on your readings.

From our pharmacist’s overview on the Dasiglucagon Hydrochloride page

“Dasiglucagon (Zegalogue) is an emergency injection that rapidly raises blood sugar during a severe diabetic low, so a supplement list this large deserves genuine care. All 239 interacting ingredients act through blood glucose, and most of them lower it, deepening the very crashes this drug exists to reverse.”

About Black Pepper

What is Black Pepper?

Black Pepper
  • Flavoring and culinary use
  • Digestive complaints like bloating and indigestion
  • Boosting absorption of other supplements
  • Antioxidant and anti-inflammatory support
  • Cold and congestion remedies

Black pepper is a common kitchen spice that is generally safe in the amounts used in food. Its extract, piperine, is mostly added to supplements to help the body absorb other ingredients (like curcumin from turmeric). Strong human evidence for black pepper treating specific health conditions is limited, so use supplement-strength products with care and check with your pharmacist.
