The piece of this combination product I want to focus on is the theophylline, since that is what black pepper can interact with. Piperine, the active part of black pepper, seems to help your body absorb theophylline and then clear it more slowly. The likely result is that theophylline levels creep up a bit higher than expected.

Theophylline is a drug where small changes matter, so even a modest bump could show up as jitteriness, a racing or pounding heartbeat, nausea, trouble sleeping, or feeling shaky. A sprinkle of pepper on dinner is very unlikely to cause a problem. The bigger concern is concentrated piperine supplements (often paired with turmeric or curcumin). This concern is based on pharmacology rather than strong studies, so don't panic, but do run it by your pharmacist or doctor before combining them.

Theoretically, black pepper might decrease levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research suggests that black pepper induces CYP1A2. This has not been reported in humans.

This combination product contains theophylline, which your liver clears using an enzyme called CYP1A2. Lab studies hint that black pepper (really its active compound, piperine) might speed that enzyme up. If that happened in people, your body could clear theophylline a little faster, which in theory might make it work less well (think more wheezing or breathing trouble that isn't fully controlled).

Here's the important part: this is only seen in test-tube research, not in actual patients, so I wouldn't lose sleep over it. The pinch of pepper on your dinner is a non-issue. Concentrated piperine supplements are where I'd be more cautious. Since theophylline has a narrow safe range, just run any pepper-extract supplement past your pharmacist or doctor first.
