Saquinavir falls under P-GLYCOPROTEIN SUBSTRATES — the group Black Pepper interacts with.
In vitro research shows that piperine, a constituent of black pepper, seems to inhibit P-glycoprotein.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for saquinavir (Invirase). Your body uses a kind of cellular pump called P-glycoprotein to push this medicine back out of your gut and limit how much gets absorbed. Piperine, the active part of black pepper, seems to slow that pump down (at least in lab studies). If the pump works less, a bit more saquinavir could stay in your blood, which in theory could mean stronger effects and more side effects like nausea, diarrhea, belly upset, or headache.
I'll be honest though, this is theoretical and based only on lab research, not real patient data. The pinch you sprinkle on food almost certainly won't matter. Concentrated piperine supplements are the bigger question. Since HIV therapy is so important, just run any black pepper supplement by your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

In vitro research and pharmacokinetic simulation data suggest that piperine, a constituent of black pepper, as well as the pepper fruit seem to inhibit CYP3A4. This has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for saquinavir (Invirase). Your body relies on a liver enzyme called CYP3A4 to clear this medicine. The piperine in black pepper can slow that enzyme down, at least in lab and computer-model studies. If it does the same thing in people, less saquinavir would get broken down, so more could linger in your blood. That could mean stronger effects and a higher chance of side effects like nausea, diarrhea, belly upset, or changes in blood sugar and fats.
The reassuring part: this is only theoretical and hasn't been shown in actual humans. A sprinkle of pepper on your food is very unlikely to matter. Concentrated piperine supplements are where I'd be cautious, so run those by your pharmacist or doctor first.

Saquinavir is used in combination with ritonavir (Norvir) and other medications to treat human immunodeficiency virus (HIV) infection. Saquinavir is in a class of medications called protease inhibitors. It works by decreasing the amount of HIV in the blood. Although saquinavir does not cure HIV, it may decrease your chance of developing acquired immunodeficiency syndrome (AIDS) and HIV-related illnesses such as serious infections or cancer. Taking these medications along with practicing safer sex and making other lifestyle changes may decrease the risk of transmitting the HIV virus to other people.
