Trazodone falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Black Pepper interacts with.

Theoretically, black pepper might increase levels of drugs metabolized by CYP2D6.
In vitro research suggests that some constituents of black pepper inhibit CYP2D6. This has not been reported in humans.

Here's the gist for your trazodone: piperine, the active part of black pepper, may slow down a liver enzyme called CYP2D6, which helps clear trazodone from your body. If that enzyme is blocked, a bit more trazodone could hang around than usual. In plain terms, that might mean stronger effects like extra drowsiness, dizziness, dry mouth, or feeling lightheaded when you stand up. Trazodone is not a prodrug, so it isn't the flipped situation where you'd lose effect.

The reassuring part: this is only a theory from lab dish studies, and it hasn't actually been seen in real people. The sprinkle of pepper on your dinner is no concern at all. Concentrated piperine supplements are where I'd just give your pharmacist a quick heads-up.

In vitro research and pharmacokinetic simulation data suggest that piperine, a constituent of black pepper, as well as the pepper fruit seem to inhibit CYP3A4. This has not been reported in humans.

Here's the gist for trazodone. Your liver uses an enzyme called CYP3A4 to clear it from your body. Piperine, the active part of black pepper, can slow that enzyme down (at least in lab and computer-modeled studies). If it slows clearance, a bit more trazodone could linger in your system, which might mean more of its usual effects like drowsiness, dizziness, dry mouth, or feeling lightheaded when you stand up.

The reassuring part: this has only been seen in test-tube and simulation research, never confirmed in real people, and pepper sprinkled on your food is very unlikely to matter. Concentrated piperine supplements (often paired with turmeric) are where I'd pause. If you take one regularly, just run it by your pharmacist or doctor first.
