Ranolazine falls under ORAL DRUGS — the group Black Psyllium interacts with.

Theoretically, psyllium might increase, decrease, or have no effect on the absorption of oral drugs.
Psyllium seems to have variable effects on drug absorption. To avoid changes in absorption, take psyllium 30-60 minutes after oral medications. Animal research shows that blond psyllium delays and increases the absorption of metformin and ethinyl estradiol. Case reports and animal research suggest that blond psyllium might reduce absorption of lithium, digoxin, olanzapine, and carbamazepine. Finally, some pharmacokinetic studies show that psyllium does not affect the absorption of levothyroxine or warfarin. Although many of these studies evaluated blond psyllium, the fiber content in black psyllium may have similar effects.

Here's the simple version: when you swallow black psyllium, its soluble fiber turns into a thick gel in your gut. That gel can sometimes grab onto medications and slightly change how much gets absorbed. With ranolazine (Ranexa), this is mostly a theoretical concern. We don't have studies showing a real problem, and the interaction is rated minor.

Ranolazine isn't a prodrug, so the worry would simply be the fiber blunting its absorption, which could in theory make it a little less effective for your angina (chest pressure). An easy fix is timing: take your ranolazine first, then wait about 30 to 60 minutes before your psyllium. If you notice more chest discomfort, check in with your pharmacist or doctor.

Black psyllium is a soluble-fiber supplement made from the seeds of a Plantago plant, used mostly to ease constipation and support digestive health. It is best known and most studied for relieving constipation and may modestly help lower cholesterol, but it must always be taken with plenty of water. What Is Ranolazine Used For?

Brand names: Ranexa, Aspruzyo

Ranolazine treats chronic angina — the recurring chest pain or pressure that happens when the heart isn't getting enough blood and oxygen. It is used when other angina medicines aren't enough on their own.

Both the extended-release tablet (sold as Ranexa and generic ranolazine) and the oral powder form (sold as Aspruzyo Sprinkle) are approved for this use. Ranolazine can be combined with other heart medicines such as beta-blockers, nitrates, calcium channel blockers, anti-platelet drugs, statins, ACE inhibitors, and angiotensin receptor blockers.
