Risedronate falls under ORAL DRUGS — the group Black Psyllium interacts with.

Theoretically, psyllium might increase, decrease, or have no effect on the absorption of oral drugs.
Psyllium seems to have variable effects on drug absorption. To avoid changes in absorption, take psyllium 30-60 minutes after oral medications. Animal research shows that blond psyllium delays and increases the absorption of metformin and ethinyl estradiol. Case reports and animal research suggest that blond psyllium might reduce absorption of lithium, digoxin, olanzapine, and carbamazepine. Finally, some pharmacokinetic studies show that psyllium does not affect the absorption of levothyroxine or warfarin. Although many of these studies evaluated blond psyllium, the fiber content in black psyllium may have similar effects.

Here's the practical concern with risedronate (Actonel DR) and black psyllium fiber. Risedronate is already a notoriously picky drug when it comes to absorption. Very little of each dose actually gets into your body, and anything in your stomach (food, minerals, or fiber) can grab onto it and block what little does absorb. The thick gel that psyllium forms could trap risedronate and lower how much reaches your bones.

The result wouldn't be more side effects, it would be less benefit, meaning weaker protection for your bones. The evidence here is theoretical and graded as minor, so don't panic. Risedronate already must be taken on an empty stomach with plain water, well away from other things. Ask your pharmacist about the best timing so your fiber and your bone medicine don't collide.

Black psyllium is a soluble-fiber supplement made from the seeds of a Plantago plant, used mostly to ease constipation and support digestive health. It is best known and most studied for relieving constipation and may modestly help lower cholesterol, but it must always be taken with plenty of water. Talk with your pharmacist or doctor before starting, especially if you have swallowing problems or bowel disorders.

What Is Risedronate Used For?

Brand name: Actonel DR

Risedronate sodium tablets are used to treat and prevent osteoporosis in postmenopausal women, treat osteoporosis in men to increase bone mass, and treat and prevent osteoporosis caused by long-term steroid (glucocorticoid) use in both men and women. They are also used to treat Paget's disease of bone — a condition where bones break down and rebuild abnormally, becoming enlarged and weakened.

Risedronate sodium delayed-release tablets (brands Atelvia and Risedronate Sodium delayed-release) are approved specifically for the treatment of postmenopausal osteoporosis only — not for the other uses listed above.
