Levomethadyl (Orlaam) is a powerful, long-acting opioid that was used to treat opioid dependence. Like other opioids, it slows down the central nervous system, which can cause drowsiness, slowed breathing, and low blood pressure. Black seed has shown CNS depressant effects in lab and animal studies, so the concern here is that the two together could add up and leave you more sedated than expected.

I want to be honest about the strength of this: it is theoretical, based on animal data, not human studies, so we don't know how big the effect really is. Food amounts of black seed as a spice are very unlikely to matter. Still, with a potent opioid like this, it's worth a quick chat with your doctor or pharmacist before adding a concentrated supplement.
