Remifentanil is a powerful, fast-acting opioid given through an IV during surgery or procedures, and one of its main effects is slowing down the brain and breathing. Black seed isn't broken down by the same pathways here. Instead, the concern is simpler: animal studies hint that black seed may have its own mild calming, sedative-like effect on the nervous system. Stacked on top of an opioid, that could in theory add up to more drowsiness or slower, shallower breathing.

Honestly, this is a theoretical worry based only on lab and animal data, not human studies, so I wouldn't lose sleep over it. Since remifentanil is only given in a monitored setting, just tell your anesthesia or surgical team about any black seed supplements you take beforehand.
