Aspirin, Codeine Phosphate, Phenobarbital falls under ORGANIC ANION TRANSPORTER 3 (OAT3) SUBSTRATES — the group Caffeic Acid interacts with.
In vitro, caffeic acid inhibits OAT3. This drug transport protein is involved in renal tubular uptake of some drugs from the blood and then elimination in the urine. Inhibition of this transporters decreases renal elimination and increases drug levels in the body. Although caffeic acid inhibits OAT3 in vitro, the clinical significance of this in humans is not known.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combination pill pairs aspirin, codeine, and phenobarbital. The piece that connects to caffeic acid here is your kidneys. There's a kidney transporter called OAT3 that helps move certain drugs out of your blood and into your urine. In lab tests (not human studies), caffeic acid appears to block OAT3 a bit. If that happened in your body, drug levels could creep up instead of leaving as quickly.
For this medicine, slightly higher levels could mean more ringing in the ears or stomach upset from the aspirin, or more drowsiness and slowed breathing from the codeine and phenobarbital. That said, this is only test-tube evidence, and we don't know if it matters in real people. Coffee-level amounts of caffeic acid are unlikely to be an issue, but before adding a concentrated supplement, please check with your pharmacist or doctor.

This combination pill blends aspirin, codeine, and phenobarbital. The piece that matters here is the aspirin, since your kidneys clear it partly through a transporter called OAT1. In a lab dish, caffeic acid blocks that transporter, which in theory could slow aspirin's exit and let a bit more linger in your body. If that happened, you might notice more aspirin-type effects like stomach upset, easy bruising, or ringing in the ears.
But here's the honest part: this is only seen in test-tube studies, not in real people, so we genuinely don't know if it matters at normal doses. Coffee-level amounts of caffeic acid are very unlikely to be an issue. If you're considering a concentrated supplement, run it past your pharmacist or doctor first.
