Here's the basic idea with these two. Thioridazine (Mellaril) has some anticholinergic activity, meaning it dampens a nerve signal called acetylcholine, which can lead to things like dry mouth, constipation, blurry vision, and a fast heartbeat. Calabar bean contains physostigmine, which does the opposite, it makes acetylcholine pile up. So on paper, the bean could push back against and weaken some of thioridazine's anticholinergic effects.

The good news is this is purely a theoretical concern based on how these chemicals behave, not real patient reports, and the interaction is rated minor and unlikely. The bigger issue is that Calabar bean is genuinely poisonous and is not a safe supplement to take at all. Please don't use it, and check with your doctor or pharmacist before adding anything new alongside thioridazine.

What Is Thioridazine Used For?

Brand name: Mellaril

Thioridazine Hydrochloride tablets are used for the management of schizophrenia — but only in patients who have already tried at least two other antipsychotic medications without adequate results, either because those drugs didn't work well enough or caused intolerable side effects.

Because thioridazine carries serious heart-rhythm risks, it is considered a last-resort option for schizophrenia, not a first-line treatment. It is not approved for dementia-related psychosis or any other psychiatric condition.
