Here's the gist for Tracleer (bosentan). The concern isn't about how your liver breaks the drug down. It's about overlapping effects on your blood pressure. Bosentan already relaxes and widens blood vessels to ease the strain in your lungs, and in animal studies calamus appeared to slow the heart and lower blood pressure too. Stack the two and, in theory, your pressure could drop more than expected, leaving you dizzy, lightheaded, or unusually tired.

I'll be honest: this is based on lab and animal work only, not human studies, so it's more caution than alarm. That said, calamus carries its own safety baggage and is generally one to avoid. Please loop in your doctor or pharmacist before mixing them, and don't change your bosentan on your own.

Bosentan also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, taking calamus with drugs metabolized by CYP3A4 might increase drug levels and potentially increase the risk of adverse effects.
In vitro research shows that calamus extract inhibits CYP3A4 enzyme.

Here's the basic idea with bosentan (Tracleer). Your liver uses an enzyme called CYP3A4 to help clear bosentan out of your system. Lab studies suggest calamus may slow that enzyme down. If it does, bosentan could build up a bit more than usual, which might raise the odds of side effects like headache, flushing, swelling in the legs or ankles, low blood pressure, or changes in your liver tests (something your doctor already watches closely with this medicine).

I'll be honest with you: this warning comes only from test-tube research, not from studies in people, so it's theoretical. On top of that, calamus contains beta-asarone, which has safety concerns of its own, so it's a supplement I'd steer clear of anyway. Please check with your pharmacist or doctor before mixing the two.

Calamus is a swamp plant with a long history in Ayurvedic and traditional Chinese medicine, mostly for digestive and nervous-system complaints. However, it contains beta-asarone, a compound linked to cancer and other toxic effects in animal studies, which is why the U.S. FDA banned it from foods. Because of these safety concerns and a lack of solid human evidence, it is generally best avoided.

What Is Bosentan Used For?

Brand name: Tracleer

Bosentan is used to treat pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs. In adults, it helps improve the ability to exercise and slows how quickly the disease gets worse.

In children aged 3 years and older with idiopathic (no known cause) or congenital (present from birth) PAH, bosentan is used to lower resistance in the lung blood vessels, which is expected to improve exercise ability. Tracleer is one brand approved for both adults and pediatric patients with PAH.
