Hydrocodone, Ibuprofen falls under CNS DEPRESSANTS — the group Calamus interacts with.

Vicoprofen combines hydrocodone, an opioid pain reliever, with ibuprofen. The opioid part already calms down your central nervous system, which is why it can make you feel drowsy, foggy, or slow your breathing. The concern here is with the calamus, not the ibuprofen side.

In animal studies, calamus appears to act as a mild sedative by raising a calming brain chemical called GABA. So in theory, pairing it with hydrocodone could pile on the sleepiness, leaving you extra groggy, dizzy, or unsteady. To be clear, this is a theoretical interaction based on lab and animal data, not proven in people. Honestly, calamus is best avoided anyway because of its own safety concerns. Please check with your pharmacist or doctor before combining them.

Theoretically, taking calamus with drugs metabolized by CYP2D6 might increase drug levels and potentially increase the risk of adverse effects.
In vitro research shows that calamus extract inhibits CYP2D6 enzyme.

Here's the wrinkle with Vicoprofen (which combines hydrocodone with ibuprofen): the hydrocodone part is actually a prodrug. Your body uses an enzyme called CYP2D6 to convert some of it into hydromorphone, the form that does much of the pain-relieving work. Lab studies suggest calamus may slow that same enzyme down. In theory, that could mean less of the active painkiller is formed, so you may get weaker pain relief.

I want to be honest though: this is based only on test-tube research, not real people, so the actual effect is uncertain. The bigger concern with calamus is its own toxicity, since the FDA banned it from foods. I'd skip calamus and run any combo past your pharmacist or doctor first.

Vicoprofen combines hydrocodone (an opioid painkiller) with ibuprofen. The hydrocodone part relies partly on a liver enzyme called CYP3A4 to be processed. In lab dish studies, calamus appears to slow that enzyme down. In theory, that could let more hydrocodone hang around in your body, which might mean stronger effects like heavy drowsiness, dizziness, constipation, nausea, or slowed breathing.

I want to be honest with you though: this is only test-tube evidence, not something proven in people, so the real-world risk is uncertain. On top of that, calamus contains beta-asarone, which has raised serious safety concerns, so it's a supplement I'd generally steer clear of. Please check with your pharmacist or doctor before combining the two.

Calamus is a swamp plant with a long history in Ayurvedic and traditional Chinese medicine, mostly for digestive and nervous-system complaints. However, it contains beta-asarone, a compound linked to cancer and other toxic effects in animal studies, which is why the U.S. FDA banned it from foods. Because of these safety concerns and a lack of solid human evidence, it is generally best avoided.
