Ephedrine Sulfate, Hydroxyzine, Theophylline falls under THEOPHYLLINE — the group Capsicum interacts with.

Theoretically, taking capsicum with theophylline might increase the levels and adverse effects of theophylline.
In animal research, oral administration of capsicum reduced excretion of theophylline. However, capsicum does not seem to affect the pharmacokinetics of theophylline when administered intravenously.

The piece of your combo medicine I want to focus on here is the theophylline, since that is where capsicum (chili pepper) could matter. In one animal study, capsicum slowed how much theophylline the body got rid of, which could let theophylline levels creep up. Theophylline is a tricky drug because it has a narrow safety window, so even a small bump can show up as a racing or pounding heart, jitteriness, nausea, trouble sleeping, or in larger amounts, tremors or seizures.

That said, this is only based on animal lab work, and it did not happen when theophylline was given by IV. So this is more of a theoretical caution than a proven problem, and an occasional spicy meal is unlikely to matter. Concentrated capsicum capsules are the bigger question mark. Please check with your pharmacist or doctor before adding a supplement.
