Capsicum + Exenatide
Exenatide brand names: Byetta, Bydureon.
Capsicum has a moderate interaction with Exenatide. Theoretically, taking capsicum with antidiabetes drugs might increase the risk of hypoglycemia.

Here is the interaction we found between these two, with the evidence behind it.
ANTIDIABETES DRUGS
Why this applies to Exenatide
Exenatide falls under ANTIDIABETES DRUGS — the group Capsicum interacts with.
Preliminary clinical research shows that consuming capsicum 5 grams along with a glucose drink attenuates the rise in plasma glucose after 30 minutes by 21%, decreases the 2-hour postprandial area under the curve of plasma glucose by 11%, and increases the 2-hour postprandial area under the curve of plasma insulin by 58% in healthy individuals when compared with placebo. Other clinical research shows that taking capsicum 5 mg daily for 28 days significantly reduces postprandial blood glucose and insulin levels, but not fasting blood glucose and insulin levels, in patients with gestational diabetes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist when you pair chili pepper supplements with exenatide (Byetta or Bydureon). Exenatide already lowers your blood sugar by helping your pancreas release insulin when sugar is high, and capsicum may nudge blood sugar down a little too. Stack them and you could see low blood sugar (hypoglycemia) more easily, with symptoms like shakiness, sweating, dizziness, a racing heart, or feeling confused.
I want to keep this in perspective. The evidence here is on the weaker side and mostly theoretical, and the chili you eat in normal meals is unlikely to cause trouble. Concentrated oral capsicum supplements are the bigger question mark. If you'd like to try one, check your blood sugars more closely at first and chat with your pharmacist or doctor before starting.

From our pharmacist’s overview on the Exenatide page
“Because exenatide (Byetta, Bydureon) exists to lower blood sugar, nearly every supplement on this list pulls at that same lever, and I treat that as a genuine warning rather than a footnote. Most entries, including everyday items like fenugreek, bitter melon, flaxseed, and ginseng, can drop glucose further and raise the odds of hypogly…”

About Capsicum
What is Capsicum?
- Nerve pain (neuropathy)
- Muscle and joint pain (arthritis)
- Back pain
- Digestive support
- Cluster headache (intranasal)
Capsicum (chili pepper) contains capsaicin, which is best known and best studied as a topical treatment for certain types of pain. Topical capsaicin products are supported by reasonable evidence for some nerve and joint pain, but oral supplements are far less proven. Use it carefully, follow product directions, and talk to your pharmacist or doctor before starting.
About Exenatide Injection
What Is Exenatide Injection Used For?
Brand names: Byetta, Bydureon
Exenatide injection is used as an addition to diet and exercise to improve blood sugar control in adults with type 2 diabetes. It is not a substitute for healthy lifestyle changes — it works best when combined with them.
It should not be used together with other exenatide-containing products. The brands Byetta, Bydureon, and BYDUREON BCise all contain exenatide and should never be combined with each other.

Exenatide Is Linked to Lower Levels of One Nutrient
Natural Medicines records Exenatide as a medicine that can lower these. This is not an interaction with Capsicum — it is a separate effect of the medicine, and it is a reason some people are advised to supplement rather than to avoid one:
