Hydrochlorothiazide, Methyldopa falls under DIURETIC DRUGS — the group Caraway interacts with.

Theoretically, caraway might increase the risk of hypokalemia when used with diuretics that deplete potassium.
Animal research suggests that a single dose of caraway fruit extract can promote diuresis and increase the urinary excretion of sodium and potassium. However, sub-chronic use of caraway fruit extract does not seem to significantly increase potassium excretion, although urine output continues to be increased for up to 6 days.

Your combo pill pairs hydrochlorothiazide, a water pill, with methyldopa for blood pressure. The piece that interacts with caraway here is the hydrochlorothiazide, which already nudges your body to flush out potassium along with extra fluid. The thought is that concentrated caraway (the oil or extract, not the spice on your rye bread) may act as a mild diuretic too, so in theory the two together could pull potassium a little lower.

I'll be honest, this concern is theoretical and based mostly on animal data, and interestingly the same research showed potassium loss faded with repeated use. Low potassium can cause muscle cramps, weakness, or an irregular heartbeat, so it's worth a quick chat with your pharmacist before starting caraway supplements. Culinary amounts shouldn't be an issue.
