Here's the gist with carob and Zokinvy (lonafarnib). Carob is loaded with fiber and tannins, and those can grab onto things in your gut. The concern is mostly about absorption: if carob and your medicine are sitting in your stomach at the same time, the carob could bind some of the drug and keep your body from taking in the full dose. For lonafarnib, getting less than the intended amount matters, since it's prescribed at a careful dose to slow progeria.

Now, the honest part: this is a theoretical worry based only on lab studies, mostly about minerals, not a proven problem with lonafarnib itself. A simple fix is spacing them out, taking carob about 30 to 60 minutes after your medicine. Since lonafarnib is so specialized, please run this by your pharmacist or doctor first.

Carob is a Mediterranean tree whose pods are widely used as a caffeine-free, naturally sweet cocoa alternative and as a thickener (carob gum). It is mostly eaten as a food and is generally well tolerated, and carob pod preparations have some traditional and limited research use for diarrhea, but evidence for most medicinal claims is limited. Check with your pharmacist or doctor before using it as a treatment, especially for infants or children.

What Is Lonafarnib Used For?

Brand name: Zokinvy

Zokinvy (lonafarnib) is approved for patients 12 months of age and older who have a body surface area of 0.39 m² or above. It is used to reduce the risk of mortality in Hutchinson-Gilford Progeria Syndrome (HGPS) — a rare, severe premature-aging disorder.

Zokinvy is also used to treat processing-deficient Progeroid Laminopathies — a related group of rare genetic conditions — specifically those caused by a heterozygous LMNA mutation with progerin-like protein buildup, or by homozygous or compound heterozygous ZMPSTE24 mutations. It is not indicated for other Progeroid Syndromes or processing-proficient Progeroid Laminopathies.
