Here's the gist with carob and your promethazine (Phenergan): the worry isn't that carob changes how your body processes the drug, it's that carob is full of fiber and tannins that can grab onto things in your stomach. In a test-tube and infant-formula setting, that binding lowered how much of certain minerals got absorbed. The thought is it might do the same to some oral pills, meaning a little less promethazine could get into your system and it may not work quite as well for your nausea, allergies, or sleep.

Honestly, this is theoretical and rated minor, based only on lab work, so I wouldn't lose sleep over it. A simple fix is to take your promethazine first, then enjoy carob 30 to 60 minutes later. If you use carob daily or in large amounts, mention it to your pharmacist.

Carob is a Mediterranean tree whose pods are widely used as a caffeine-free, naturally sweet cocoa alternative and as a thickener (carob gum). It is mostly eaten as a food and is generally well tolerated, and carob pod preparations have some traditional and limited research use for diarrhea, but evidence for most medicinal claims is limited. Check with your pharmacist or doctor before using it as a treatment, especially for infants or children.

About Promethazine

What Is Promethazine Used For?

Brand name: Phenergan

Promethazine is used for a wide range of conditions. Tablets, oral solution, and rectal suppositories (brands including Phenergan, Promethegan, and Phenadoz) are approved for seasonal and perennial allergic rhinitis, vasomotor rhinitis, allergic conjunctivitis, hives (urticaria), angioedema, dermographism, and allergic reactions to blood or plasma. They are also used for motion sickness (both prevention and active treatment), nausea and vomiting related to anesthesia and surgery, sedation, and as a helper medication alongside pain relievers after surgery.

Promethazine hydrochloride injection (brands including Phenergan and Promethazine HCl injection) is used for many of the same purposes but is reserved for situations where oral treatment is not possible — including allergic emergencies as an adjunct to epinephrine, sedation during labor, and as an aid during certain surgical procedures such as bronchoscopy and eye surgery.
