Diltiazem falls under ANTIHYPERTENSIVE DRUGS — the group Coca interacts with.

Theoretically, coca might reduce the effects of antihypertensive drugs.
Some clinical research suggests that coca and the coca constituent cocaine have hypertensive effects. Theoretically, concomitant use of coca with antihypertensive drugs might interfere with blood pressure control and decrease the effectiveness of antihypertensive drugs.

Here's the thing with coca and your diltiazem: the two are working against each other. Diltiazem is doing its job by relaxing your blood vessels so your heart doesn't have to work so hard, which lowers your blood pressure and eases chest pain. Coca, on the other hand, contains cocaine and related stimulant compounds that tend to raise blood pressure and rev up your heart rate. So coca can blunt the benefit you're getting from diltiazem.

In real terms, your blood pressure could creep up or your angina could feel less controlled. This is mostly theoretical and based on lower quality evidence, but the direction makes sense. Please don't change anything on your own. Just check with your pharmacist or doctor before using coca.

Coca is a South American plant best known as the natural source of cocaine. While the leaves have a long history of traditional use for altitude sickness, fatigue, and hunger, the whole plant contains cocaine and other alkaloids that make it legally controlled and potentially dangerous in most countries. Only decocainized (cocaine-free) coca extract is used in foods, and there is little reliable evidence to recommend coca as a supplement.

Brand names: Cardizem, Cardizem SR, Diltiaz SR, Cardizem CD, Tiazac, Diltiaz XT, Diltiaz CD, Dilacor, Diltiaz, Cardizem LA, Dilacor XR, Cartia XT, Tiamate, Adizem-SR, Adizem-XL, Angitil SR, Angitil XL, Calcicard CR, Dilcardia SR, Dilzem SR, Dilzem XL, Slozem, Tildiem LA, Tildiem Retard, Viazem XL, Zemtard 120XL, Zemtard 180XL, Zemtard 240XL, Zemtard 300XL

Oral extended-release forms of diltiazem — including Matzim LA, Cardizem LA, Cartia, Cartia XT, Tiadylt ER, and various generic capsules and tablets — are used to treat high blood pressure (hypertension) and chronic stable angina (chest pain brought on by activity). Some capsule formulations, including Cartia, Cartia XT, and certain generics, are also approved for angina due to coronary artery spasm (sudden chest pain caused by a sudden tightening of a coronary artery).

Injectable forms of diltiazem are used in hospital settings to temporarily slow a dangerously fast heart rate in atrial fibrillation or atrial flutter (rapid, irregular heart rhythms), and to restore a normal heart rhythm in paroxysmal supraventricular tachycardia (PSVT) — sudden episodes of a very fast but regular heartbeat originating above the heart's lower chambers.
