None of the listed interactions are rated major, and the single concern behind them rests on animal research that has not been confirmed in people.
The one area worth watching is diabetes medications. Animal studies suggest Terminalia bellirica fruit extract may lower blood sugar, so combining it with drugs that already do this could, in theory, push blood sugar too low. Because blood sugar is a closely monitored measure, people taking these medicines should keep an eye on their readings and mention the supplement at their next check-up. No human studies have shown this effect so far.
Although the number of medications flagged looks large, nearly all of it traces back to this one theoretical blood sugar concern applied across many individual drugs. In other words, a long list does not mean dozens of separate proven problems. The predicted effect comes from animal research and has not been shown to cause meaningful changes in people.

Antidiabetes Drugs

Overview
Terminalia bellirica is a large deciduous tree native to South and Southeast Asia, including India. Its dried, ripe fruit is the part most often used in traditional medicine. In Ayurveda, the fruit is commonly known as 'Bibhitaki' or 'Vibhitaki.'
It is best known as one of the three fruits in Triphala, a classic Ayurvedic blend that also contains Terminalia chebula and Emblica officinalis (amla). People take Terminalia bellirica as a powder, in capsules or tablets, or as a tea-like decoction.
Traditionally it has been used for digestion, breathing problems, and overall wellness. Today it is also marketed as an antioxidant and metabolic-support supplement.
How it works
The fruit contains plant compounds such as tannins, gallic acid, ellagic acid, and other polyphenols. In laboratory and animal studies, these compounds show antioxidant activity, meaning they may help neutralize unstable molecules called free radicals.
Researchers have also looked at possible effects on inflammation, certain enzymes, fat metabolism, and microbes in test-tube settings. These findings are early and do not prove the same effects happen in the human body.
It is important to remember that mechanisms seen in a lab or in animals often do not translate directly to real benefits in people.

Terminalia bellirica has a long history of use in foods and traditional medicine, which is reassuring, but well-designed human safety studies are limited. The quality and purity of supplements can vary, and some products may be contaminated with heavy metals or other substances.
People with digestive conditions, those scheduled for surgery, or anyone with a chronic illness should talk to a healthcare provider first. Because the fruit may affect blood sugar, people with diabetes should monitor closely.

Side effects
When taken in typical amounts, Terminalia bellirica is generally well tolerated by many people, but solid safety data are limited.
Possible side effects can include digestive upset such as loose stools, diarrhea, gas, or stomach discomfort, especially at higher doses or when used as a laxative-type remedy.
Rarely, some people may have an allergic reaction. Stop using it and seek medical care if you notice rash, swelling, trouble breathing, or other signs of a serious reaction.

In a small clinical study, two adults with hyperlipidemia developed a hypersensitivity rash, one with facial edema, within minutes of oral administration of Triphala (containing an aqueous extract of Terminalia chebula, Terminalia bellirica, and Indian gooseberry), together with guggul extract. It is unclear if this reaction was due to Triphala, guggul, or other factors.

Dosing
There is no well-established standardized dose for Terminalia bellirica. Traditional use and supplement products vary widely in the amount and form used.
The best advice is to follow the directions on the product label and not exceed the recommended amount. Because formulas (powder, capsule, Triphala blend, extract) differ in strength, doses are not interchangeable.
