Animal research suggests that Terminalia bellirica fruit extract, alone or in combination with Terminalia chebula and Indian gooseberry, has hypoglycemic effects. This activity has not been shown in humans.

Here's the simple version: metformin already works to lower your blood sugar, and some animal studies hint that Terminalia bellirica (Bibhitaki) may nudge blood sugar down too. If both pull in the same direction, your numbers could theoretically dip lower than expected. That's worth knowing, but I want to be honest with you: this is based on lab and animal work, not solid human studies, so it's more of a 'keep an eye out' than a red alert.

If you do try this supplement alongside metformin, watch for signs of low blood sugar like shakiness, sweating, dizziness, or feeling unusually hungry or foggy. Check your readings a bit more often at first, and please loop in your pharmacist or doctor before starting it so they can guide you safely.
