Animal research suggests that Terminalia bellirica fruit extract, alone or in combination with Terminalia chebula and Indian gooseberry, has hypoglycemic effects. This activity has not been shown in humans.

Toujeo (insulin glargine) is your long-acting background insulin, doing a steady job of lowering blood sugar all day. Here's the thing to keep in mind with Bibhitaki: in animal studies, this fruit appeared to lower blood sugar on its own. If that effect also happens in people, stacking it on top of your insulin could push your blood sugar lower than you intend.
That means a slightly higher chance of hypoglycemia, which can show up as shakiness, sweating, dizziness, confusion, or a fast heartbeat. I want to be honest, though: this is a theoretical concern based on lab and animal work, not proven in humans. If you'd like to try it, talk with your doctor or pharmacist first and watch your glucose readings more closely at the start.
