Here's the thing to know about coffee charcoal: like other charcoal-type products, it has a porous surface that can grab onto things in your gut, including medicine you swallow. If it binds to a drug before your body absorbs it, you end up getting less of that medicine than intended.

The good news for carmustine (BiCNU, Gliadel) is that it's given as an IV infusion or as a wafer placed directly in the brain during surgery, not as a pill you take by mouth. So this particular concern, which is about oral drugs, really doesn't apply to how carmustine is normally given. This worry is also theoretical, based on how charcoal works rather than real-world reports. Still, since carmustine is serious cancer treatment, please run any supplement past your oncology team first.

What Is Carmustine Used For?

Brand names: Gliadel, BiCNU, Gliadel Wafer

Carmustine injection is used as palliative therapy — either alone or combined with other chemotherapy drugs — for several brain tumors, including glioblastoma, brainstem glioma, medulloblastoma, astrocytoma, ependymoma, and metastatic brain tumors. It is also used for multiple myeloma (in combination with prednisone), and for relapsed or refractory Hodgkin's lymphoma and Non-Hodgkin's lymphomas (in combination with other approved drugs).

Gliadel — a dissolving wafer form of carmustine placed directly into the brain during surgery — is specifically indicated for newly diagnosed high-grade glioma (as an addition to surgery and radiation) and for recurrent glioblastoma (as an addition to surgery).
