Coffee Charcoal and Cholic Acid

Here is the interaction we found between these two, with the evidence behind it.

ORAL DRUGS

Why this applies to Cholic Acid

Cholic Acid falls under ORAL DRUGS — the group Coffee Charcoal interacts with.

Coffee charcoal can reduce the absorption of orally administered drugs and should be separated from oral drug administration by at least two hours.
Severity Moderate Likelihood Probable Evidence D

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the practical concern with pairing coffee charcoal with Cholbam (cholic acid). Charcoal-type materials are porous and tend to grab onto things in your gut, including medications. If you swallow them too close together, the charcoal may soak up some of your cholic acid before your body can absorb it, meaning less of your dose actually gets to work.

That matters because cholic acid is replacing a bile acid your body can't make properly. If absorption drops, you may not get the full benefit, which over time could let liver problems creep back. Keep in mind this is a theoretical interaction based on how charcoal behaves, not hard proof. To be safe, separate the two by at least two hours, and check with your pharmacist or your child's doctor before changing anything.

From our pharmacist’s overview on the Cholic Acid page

“Activated charcoal is the one supplement I would flatly keep away from cholic acid (Cholbam). This medicine replaces a bile acid the body cannot make on its own, and it cycles repeatedly between the gut and liver, which is exactly the loop charcoal interrupts by binding drugs in the intestine.”

About Coffee Charcoal

What is Coffee Charcoal?

  • Diarrhea
  • Inflammatory bowel conditions
  • Irritable bowel syndrome (IBS)
  • Mild digestive upset
  • Wound and skin care (topical, traditional)

Coffee charcoal is made by roasting green coffee beans until they turn black, creating a charcoal-like material used traditionally for diarrhea and digestive complaints. Scientific evidence supporting its benefits is very limited, and it should not replace proven treatments. Talk with your pharmacist or doctor before using it, especially if you have a chronic gut condition.
