Here's the simple version: coffee charcoal acts a bit like other charcoals, with a porous surface that can grab onto things in your gut. If you swallow it around the same time as your ketoconazole tablet, it may bind some of the drug before your body can absorb it. That means less ketoconazole gets into your bloodstream, and a weaker dose could let the fungal infection linger instead of clearing up.
The good news is this is based on how charcoal behaves, not on strong human studies, so it's a precaution rather than a crisis. An easy fix is timing: keep at least two hours between them. Before pairing these, run it by your pharmacist or doctor first.

What Is Ketoconazole Used For?

Ketoconazole 2% shampoo and foam are used to treat tinea versicolor (also called pityriasis versicolor) — a fungal infection caused by Pityrosporum orbiculare (also known as Malassezia furfur), a yeast that naturally lives on the skin.
This infection creates light or dark patches, most often on the trunk, neck, arms, and upper thighs. Even after the fungus is cleared, those skin patches may take months to return to their normal color — that's normal, and depends on your skin type and sun exposure. Tinea versicolor can also come back, since the fungus lives on healthy skin even when there's no active infection.
