Here's the simple version: coffee charcoal acts a bit like a sponge in your gut. Because of its porous, charcoal-like surface, it can grab onto medicines you swallow and keep some of them from being absorbed. If that happens with an oral form of mechlorethamine, you might get less of the drug into your body than your cancer team intended, which could mean it doesn't work as well as it should.

This is a theoretical concern based on how charcoal behaves, not strong proof from studies. Still, with a cancer medicine you don't want to gamble on a weaker dose. Most of the time, simply separating them by at least two hours helps. Please don't change anything on your own. Talk with your oncologist or pharmacist first, since this drug is too important to risk.
