Here's the simple version: coffee charcoal acts a bit like other charcoals in your gut. Its porous surface can grab onto things you swallow, including mefenamic acid. If they're sitting in your stomach at the same time, the charcoal may soak up some of the medicine before your body can absorb it. That means you could get less pain relief than expected for your cramps, headache, or other aches.

This isn't about mefenamic acid building up or becoming stronger. It's the opposite, you may simply get a weaker dose. The good news is the fix is easy: keep the two apart by at least a couple of hours. Honestly though, run it by your pharmacist or doctor first, since the evidence here is mostly theoretical.
