Coffee Charcoal and Midostaurin

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ORAL DRUGS

Why this applies to Midostaurin

Midostaurin falls under ORAL DRUGS — the group Coffee Charcoal interacts with.

Coffee charcoal can reduce the absorption of orally administered drugs and should be separated from oral drug administration by at least two hours.
Severity Moderate Likelihood Probable Evidence D

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: coffee charcoal acts a bit like a sponge in your gut. Because it has that porous, charcoal-like surface, it can grab onto pills you swallow and keep some of the medicine from being absorbed into your body. With Midostaurin (Rydapt), an oral leukemia and mastocytosis drug, that could mean you absorb less of it than you should, which may make it work less well against the cancer or mast cells it's meant to control.

This concern is theoretical, based on how charcoal behaves rather than studies in people taking this exact combo. Still, Midostaurin is a serious cancer drug where getting the full dose matters. Please don't change anything on your own. Talk with your oncology team or pharmacist first, and ask about separating the two by at least a couple of hours if they're okay with it at all.

From our pharmacist’s overview on the Midostaurin page

“Midostaurin (Rydapt) treats certain leukemias and mast cell disorders, and because it is a targeted cancer drug, keeping its blood level where it belongs matters enough that I would read this list carefully before adding any supplement. Almost every entry works through one liver enzyme, CYP3A4, the pathway that clears midostaurin.”

About Coffee Charcoal

What is Coffee Charcoal?

  • Diarrhea
  • Inflammatory bowel conditions
  • Irritable bowel syndrome (IBS)
  • Mild digestive upset
  • Wound and skin care (topical, traditional)

Coffee charcoal is made by roasting green coffee beans until they turn black, creating a charcoal-like material used traditionally for diarrhea and digestive complaints. Scientific evidence supporting its benefits is very limited, and it should not replace proven treatments. Talk with your pharmacist or doctor before using it, especially if you have a chronic gut condition.

About Midostaurin

What Is Midostaurin Used For?

Brand name: Rydapt

  • Midostaurin is used with other chemotherapy drugs to treat certain types of acute myeloid leukemia (AML; a type of cancer of the white blood cells). Midostaurin is also used to certain types of mastocytosis (a blood disorder in which there are too many mast cells [a certain kind of white blood cell]). Midostaurin is in a class of medications called kinase inhibitors. It works by blocking the action of the abnormal protein that signals cancer cells to multiply. This helps stop the spread of mast and cancer cells.
