Milrinone falls under ORAL DRUGS — the group Coffee Charcoal interacts with.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the thing about milrinone (Primacor): it's almost always given by IV in a hospital setting, not as a pill you swallow. The concern with coffee charcoal is that its porous, sponge-like surface can grab onto medicines sitting in your stomach and gut, keeping them from being absorbed properly. Since this only happens to oral drugs, IV milrinone wouldn't really be affected, because it bypasses the gut entirely.
I'll be honest, this caution is theoretical and based on how charcoal behaves in general, not on studies with milrinone itself. If you were somehow taking an oral form, separating it from the charcoal by at least two hours would be wise. Please run your full medication list by your pharmacist or doctor to be safe.

What Is Milrinone Used For?
Brand name: Primacor
Milrinone Lactate Injection and Milrinone Lactate in 5% Dextrose Injection are indicated for the short-term intravenous treatment of acute decompensated heart failure — a sudden or rapidly worsening episode of heart failure that requires immediate, in-hospital care.
This medication is given only in a monitored hospital setting. It is not intended for long-term or home use. Most clinical experience has been in patients who were also taking digoxin and diuretics (water pills).
