Why this applies to Zafirlukast

Zafirlukast falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Coleus interacts with.

Theoretically, taking coleus may affect drugs metabolized by CYP2C9 and increase the risk of adverse effects or reduce the effectiveness.
Research on the effect of coleus on CYP2C9 is conflicting. Some animal research shows that coleus extract can induce CYP2C9, while in vitro research shows that coleus can inhibit CYP2C9. Until more is known, advise patients that taking coleus might increase or decrease levels of drugs metabolized by CYP2C9.

Here's the situation with these two. Your body clears zafirlukast (Accolate) mostly through a liver enzyme called CYP2C9, and Coleus may tinker with that same enzyme. The catch is the lab data points in both directions: some animal studies suggest Coleus could speed the enzyme up, while test-tube studies suggest it could slow it down.

So honestly, we can't say for sure which way this would go. If Coleus slowed the enzyme, zafirlukast could build up and you might notice more side effects like headache, nausea, or stomach upset. If it sped things up, your asthma medicine might not work as well. This is all theoretical and based only on lab and animal research, so don't panic. Just loop in your pharmacist or doctor before pairing them.
