Zafirlukast falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Adrafinil interacts with.

Theoretically, drugs that inhibit CYP3A4 might increase the levels and clinical effects of adrafinil.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil is metabolized by CYP3A4.

Here's the deal with these two together. After you take adrafinil, your liver turns it into modafinil, the actual wakefulness-promoting compound. Modafinil is then cleared by a liver enzyme called CYP3A4. Zafirlukast (Accolate) can mildly block that same enzyme, so in theory it could slow the breakdown of modafinil and let it pile up a bit higher than usual.

If that happens, you might feel more of modafinil's stimulant effects: trouble sleeping, jitteriness, a racing heart, headache, or anxiety. I'll be honest though, this is a theoretical concern based on how the drugs work, not something proven in studies, so don't panic. Adrafinil is unregulated and hard on the liver on its own, so please run this combination past your doctor or pharmacist before mixing them.

Theoretically, adrafinil might increase the levels and clinical effects of CYP2C9 substrates.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to reduce CYP2C9 expression in vitro.

Here's the connection worth knowing. Your body turns adrafinil into modafinil, and modafinil may slow down a liver enzyme called CYP2C9. Since zafirlukast (Accolate) is broken down by that same enzyme, the theory is that adrafinil could let zafirlukast build up a little more than usual, leaving slightly higher levels in your system.

If that happened, you might notice more of zafirlukast's usual effects, like headache, stomach upset, or nausea. I'll be honest with you though: this is a theoretical concern based only on lab work, not real-world cases, so I wouldn't lose sleep over it. Still, since adrafinil is unregulated and can stress the liver, please run it by your pharmacist or doctor before combining the two.

Zafirlukast tablets are used for the prophylaxis (prevention) and chronic treatment of asthma in adults and children 5 years of age and older. It is taken daily to help control ongoing asthma symptoms — not to relieve a sudden asthma attack.

It is available under the brand names Accolate and Zafirlukast as an oral tablet.
