Here's the gist for this pairing. Itraconazole is one of the strongest blockers of an enzyme called CYP3A4, and your body uses that same enzyme to help break down THC. When itraconazole slows that breakdown, THC can hang around longer and build up. In plain terms, your usual amount of cannabis could hit harder than expected, with more drowsiness, dizziness, a stronger high, racy heartbeat, or feeling out of it.
Keep in mind this concern is theoretical, based on how the two work rather than on solid human studies, so it may not happen to everyone. Still, it's worth a heads-up. Don't stop your antifungal, since treating the infection matters. Just let your pharmacist or doctor know you use THC so they can guide you.

Here's the simple version: itraconazole is an antifungal, and one thing it does is block a liver enzyme called CYP2C9. That same enzyme helps your body break down THC. So when you take both, the THC may clear out more slowly and build up a bit. In plain terms, the THC effects could feel stronger or last longer than usual (more drowsiness, dizziness, a foggy head, or a faster heartbeat).
I want to be honest about the strength of this one: it's theoretical, based on how the drugs work rather than real-world reports, so it may never cause you trouble. Itraconazole itself is the medicine you don't want to mess with, since it's treating an infection. Don't change anything on your own. P-GLYCOPROTEIN SUBSTRATES

Most in vitro research suggests that THC can inhibit P-gp and increase the accumulation of probe compounds by reducing P-gp mediated drug efflux. In vitro studies in kidney cell lines show that a 1-hour exposure to CBD and THC inhibits P-gp. THC may also alter the expression of P-gp, although this effect appears to vary based on duration of exposure. Some in vitro research in lymphoblastoid leukemia cell lines indicates that a 1-hour exposure to cannabinoids does not affect P-gp expression, while a prolonged 72-hour exposure decreases P-gp expression. Other in vitro research in these cell lines shows that a 4-hour exposure to THC and CBD induces P-gp gene expression, while exposure for longer than 4 hours and up to 48 hours does not induce P-gp gene expression.

Here's the gist for itraconazole and THC. Itraconazole is carried around your body partly by a transporter pump called P-glycoprotein (P-gp), which helps move the drug in and out of cells. Lab studies hint that THC may slow this pump down. If that happened in real life, slightly more itraconazole could stay in your system, which could nudge up side effects like nausea, stomach upset, headache, or in rare cases effects on the heart or liver.
I want to be honest about the strength of this, though. This is only theoretical, based on cell and animal studies, not on people. That means we really don't know if it matters in everyday use. In vitro research shows that cannabis containing THC might modestly inhibit the activity of CYP3A4 enzymes, which might decrease the metabolism of CYP3A4 substrates. It is unclear if this effect is due to THC, other constituents, or the combination. In vitro research also shows that cannabis extracts containing THC modestly inhibit the CYP3A4 metabolism of testosterone; extracts providing the specific cannabinoids cannabidiol (CBD) or cannabigerol (CBG) alone had stronger inhibitory effects than extracts containing both THC and CBD. Conversely, a crossover clinical study in healthy adults shows that oral THC does not inhibit CYP3A4.

What Is Itraconazole Used For?
Brand names: Sporanox, Sporanox Injection, Tolsura
Itraconazole capsules (including Sporanox capsules and generic itraconazole capsules) treat serious fungal infections — blastomycosis, histoplasmosis, and aspergillosis — in both people with normal immune systems and those who are immunocompromised. Capsule forms are also approved for nail fungus (onychomycosis) caused by dermatophytes in non-immunocompromised patients. Tolsura (a gelatin-coated capsule formulation) treats blastomycosis, histoplasmosis, and aspergillosis in adults, but is not approved for nail fungus and is not interchangeable with other itraconazole products.
Itraconazole oral solution is specifically approved for oropharyngeal candidiasis (thrush — a fungal infection of the mouth) and esophageal candidiasis (a fungal infection of the food pipe). Only the oral solution has been shown to work for these mouth and throat infections; capsules cannot be substituted for this use.
