Acarbose falls under ANTIDIABETES DRUGS — the group Dragon Fruit interacts with.
Some antidiabetes drugs include glimepiride (Amaryl), glyburide (DiaBeta, Glynase PresTab, Micronase), insulin, metformin (Glucophage), pioglitazone (Actos), rosiglitazone (Avandia), and others.

Here's the simple version: dragon fruit may nudge your blood sugar down a little on its own, mostly thanks to its fiber and plant compounds. Since acarbose already works by slowing how fast sugar gets absorbed after meals, adding dragon fruit could add to that effect and make your numbers run lower than expected.
The good news is that this is based on fairly weak evidence, and acarbose by itself rarely causes low blood sugar. The bigger thing to watch is if you also take insulin or a sulfonylurea, where lows are more likely. Enjoying dragon fruit as a normal food is usually fine. If you're using concentrated extracts or notice shakiness, sweating, or dizziness, check your sugar and talk with your pharmacist or doctor before changing anything.
