This combination product already has two ingredients that can make you drowsy: the antihistamine chlorpheniramine and the opioid cough suppressant hydrocodone. (The pseudoephedrine part is actually a stimulant decongestant, so it doesn't add to the sleepiness.) The concern here is simple stacking. Elecampane may have its own mild calming effect, so taking it alongside this medicine could leave you feeling extra sleepy, foggy, dizzy, or slowed down.

I'll be honest: this is a theoretical worry based on how the herb behaves, not on strong human studies, so the real-world risk is likely small. Still, with an opioid on board, I'd play it safe. Skip driving until you know how you feel, and check with your pharmacist or doctor before combining them.
