Acetaminophen, Caffeine, Codeine falls under CNS DEPRESSANTS — the group European Barberry interacts with.
Animal research suggests that berberine, a constituent of European barberry, might have sedative effects. Theoretically, European barberry might have a similar effect.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version. This combo pill includes codeine, an opioid that can already make you drowsy and slow your breathing. The concern with European barberry is its main plant compound, berberine, which in animal studies showed mild sedative effects. So the worry is an additive one: barberry might pile a little extra sleepiness on top of what the codeine is already doing.
I want to be honest about the strength here. This is theoretical, based on pharmacology and animal data, not strong human evidence. Still, if you take this medicine, watch for extra grogginess, dizziness, or feeling foggy, and don't drive until you know how you react. Before adding a barberry supplement, please check with your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

There is very preliminary evidence suggesting that berberine, a constituent of European barberry, might inhibit the CYP3A4 enzyme. Theoretically, European barberry might have a similar effect.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combo pill contains codeine, which your body has to switch on into morphine to give you pain relief. A big part of that activation runs through a liver enzyme called CYP3A4. Barberry (through its berberine) may slow CYP3A4 down a little, so in theory more codeine could linger in its starting form and shift toward a different pathway. That could mean either less reliable pain relief or, in some folks, more codeine effects like drowsiness, constipation, or slowed breathing.
Honestly though, this is a theoretical concern based mostly on lab thinking, not strong human studies, so I wouldn't panic. A culinary pinch of barberry is unlikely to matter, but concentrated berberine supplements are worth a quick chat with your pharmacist or doctor before mixing.
