Here's the thing about nitroprusside: it's a powerful, fast-acting blood pressure medicine given through an IV in the hospital, usually under close monitoring for a hypertensive emergency. Because it already lowers blood pressure quickly, anything else that also nudges blood pressure down could add to that effect. European barberry (and its main alkaloid, berberine) has shown mild blood-pressure-lowering activity in some animal and human research, so in theory it could push your pressure lower than intended.

I'll be honest, this concern is theoretical and based on how these things work, not on strong studies in people taking them together. Realistically, since nitroprusside is given in a controlled hospital setting, your team is already watching your blood pressure closely. Still, always tell them about any supplement you take so they can keep you safe.
