Why this applies to Selinexor
Selinexor falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group European Barberry interacts with.
There is very preliminary evidence suggesting that berberine, a constituent of European barberry, might inhibit the CYP3A4 enzyme. Theoretically, European barberry might have a similar effect.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the basic idea with European barberry and Selinexor (Xpovio). Barberry contains berberine, and in early lab work berberine seems to slow down a liver enzyme called CYP3A4. Selinexor is partly cleared by that same enzyme, so in theory the barberry could let Selinexor build up a bit higher than expected in your body. Selinexor is not a prodrug, so more of it usually means stronger effects and more side effects.
For this drug that could mean worse nausea, vomiting, fatigue, low appetite, or bigger drops in your blood counts. To be clear, this is a theoretical concern based on how berberine behaves, not proven cases. Since Selinexor is serious cancer therapy, please check with your oncology team or pharmacist before adding barberry.

European barberry is a shrub whose root, bark, and berries contain berberine, a bitter plant alkaloid that has been studied mostly in isolated form. Some early research on berberine is promising for blood sugar and cholesterol, but high-quality evidence for the whole barberry herb is limited. What Is Selinexor Used For?
Brand name: Xpovio
Selinexor, sold as Xpovio tablets, treats multiple myeloma (a cancer of plasma cells in the bone marrow) in adults. It is always used together with other medicines.
Xpovio is used with bortezomib and dexamethasone in adults who have had at least one prior therapy. It is also used with dexamethasone in adults whose myeloma has come back or stopped responding after at least four prior therapies, and is resistant to several other drug classes.
