Cisapride falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group False Unicorn interacts with.

In vitro, false unicorn extract inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, false unicorn might increase the levels of CYP3A4 substrates. Drugs that might be affected include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.

Here's the concern with this pairing. Your body relies on a liver enzyme called CYP3A4 to clear cisapride out of your system. In lab tests, False Unicorn appears to slow that same enzyme down. If that happened in real life, cisapride could build up to higher levels than intended.

That matters because cisapride is a drug that was actually pulled from the regular U.S. market for a serious reason: at high levels it can throw off the heart's rhythm (a problem called QT prolongation) and cause dangerous, sometimes life-threatening, irregular heartbeats. So even a small bump in its levels is something to take seriously. The good news is this warning is based only on test-tube data, not real patients, so we don't truly know if it occurs in people. Still, given how risky cisapride is, please don't combine these without checking with your doctor or pharmacist first.
