These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
False Unicorn interacts with whole categories of medication. Fentanyl falls into 2 of those categories, so it can interact with False Unicorn in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES
1 of 2Why this applies to Fentanyl
Fentanyl falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group False Unicorn interacts with.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with False Unicorn and fentanyl. In a lab dish, this herb slowed down an enzyme called CYP2D6, which is one of the pathways your body uses to help clear fentanyl. The worry is that if that pathway is blocked, fentanyl could build up a bit more than expected. Fentanyl is an active opioid (not a prodrug), so higher levels would mean stronger effects, think more drowsiness, slowed breathing, confusion, or constipation. With a powerful drug like fentanyl, even small shifts deserve respect.
That said, this is only a test-tube finding, not something proven in real people, so I don't want to alarm you. Still, because fentanyl is so potent, please check with your pharmacist or doctor before pairing it with this supplement.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Fentanyl also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the concern with pairing False Unicorn and fentanyl. Your body relies on a liver enzyme called CYP3A4 to break down and clear fentanyl. In lab studies (test tube only, not in people), False Unicorn appears to slow that enzyme down. If that held true in the body, fentanyl could clear more slowly and build up to higher levels than intended.
With a powerful opioid like fentanyl, that matters. Higher levels can mean stronger effects like heavy drowsiness, confusion, very slow or shallow breathing, and in serious cases overdose. I want to be honest though: this is theoretical and based only on lab data, so we don't truly know if it happens in real life. Given fentanyl's risk, please run this by your doctor or pharmacist before combining them.

What These Ratings Mean
Every interaction above is graded three ways — here’s the key to each label.
Clinically significant. Avoid the combination, or use only under close professional supervision.
Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.
Generally not serious. Be aware of it and mention it to your pharmacist.
- LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
- ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
- PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
- UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
- AHigh-quality meta-analysis (quantitative systematic review).
- BCase-control study.

From our pharmacist’s overview on the Fentanyl page
“There is very little room for error with fentanyl (Duragesic, Sublimaze, Actiq, and other forms), a powerful opioid where a modest rise in blood level or an extra layer of sedation can become dangerous, so this interaction list deserves real respect. Most of the risk falls into two patterns.”

False Unicorn is a North American plant whose root has long been used in traditional medicine for women's reproductive and menstrual concerns. There is very little reliable human research to show it actually works, and safety has not been well studied, so it should be used with caution and never during pregnancy.
