Nirogacestat falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group False Unicorn interacts with.

In vitro, false unicorn extract inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, false unicorn might increase the levels of CYP3A4 substrates. Drugs that might be affected include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.

Nirogacestat (Ogsiveo), the medicine used for desmoid tumors, is cleared from your body mainly by a liver enzyme called CYP3A4. In lab (test tube) studies, False Unicorn appears to slow that same enzyme down. If that were to happen in real life, your body might clear nirogacestat more slowly, letting levels creep up. That could mean more of this drug's usual side effects, like diarrhea, nausea, mouth sores, fatigue, rash, or (in women) ovarian-related changes.

I want to be honest about the strength of this: it comes from lab data only, not from studies in people, so it's more of a caution than a proven problem. Still, nirogacestat is a serious cancer therapy, so please don't add False Unicorn without first checking with your oncologist or pharmacist.

Ogsiveo (nirogacestat) is used to treat adults with desmoid tumors that are growing or progressing and require systemic (whole-body) treatment. Desmoid tumors are rare, locally aggressive growths that arise from connective tissue.

This indication is specific to Ogsiveo tablets taken by mouth. It is not approved for use in children, as safety and effectiveness have not been established in pediatric patients.
