Why this applies to Apixaban

Apixaban falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Fennel interacts with.

Theoretically, fennel might increase levels of drugs metabolized by CYP3A4.

In vitro research suggests that fennel inhibits CYP3A4 enzyme activity. This effect has not been reported in humans.

Here's the gist with Eliquis (apixaban) and fennel. Your body partly clears apixaban using a liver enzyme called CYP3A4. In test-tube studies, fennel appears to slow that enzyme down. If that happened in real life, it could mean a bit more apixaban hanging around in your blood, which for a blood thinner means a higher chance of bleeding (easy bruising, nosebleeds, bleeding gums, or blood in your urine or stool).

The reassuring part: this is only lab-level evidence and hasn't actually been seen in people. Everyday culinary fennel in cooking or tea is very unlikely to matter. Concentrated fennel capsules or the essential oil are where I'd be more cautious. Since apixaban is a drug where small changes count, just run any fennel supplement past your pharmacist or doctor first.

Here's the simple version: Apixaban (Eliquis) is a blood thinner that lowers your blood's tendency to clot. Some animal and lab studies hint that fennel may have a mild blood-thinning effect of its own. If that holds true in people, stacking the two could add up and nudge your bleeding risk a little higher. Think easier bruising, nosebleeds, bleeding gums, or bleeding that takes longer to stop.

I want to keep this in perspective, though. This concern comes only from test-tube and animal research, not human studies, so it's theoretical. Fennel used as a food or spice is very unlikely to matter. Concentrated fennel supplements or the essential oil are where I'd be more careful. Don't change anything on your own. Just run any fennel supplement past your pharmacist or doctor first.

Fennel is a Mediterranean herb widely used as a food and spice, and traditionally taken for digestive complaints, colic, and menstrual cramps. Some small studies suggest possible benefit for certain stomach and menstrual symptoms, but the evidence is limited. It is generally well tolerated in food amounts, but concentrated supplements and the essential oil should be used with caution and after checking with your pharmacist or doctor.

About Apixaban

What Is Apixaban Used For?

Brand name: Eliquis

Apixaban is used to reduce the risk of stroke and dangerous clots traveling through the body in adults with nonvalvular atrial fibrillation (an irregular heart rhythm not caused by a heart valve problem). It is also used to prevent deep vein thrombosis (DVT — a blood clot in the leg) in adults after hip or knee replacement surgery, and to treat DVT and pulmonary embolism (PE — a blood clot in the lungs) in adults, including reducing the risk of those clots coming back.

Eliquis (the brand-name tablet and kit) is additionally approved to treat venous thromboembolism (VTE — blood clots in the veins) and reduce the risk of recurrent VTE in pediatric patients from birth and older, after at least 5 days of initial anticoagulant treatment. The generic apixaban tablet carries the adult indications only.
